Continuum Hospice Care/Jacob Perlow Hospice
Continuum Hospice Care/Jacob Perlow Hospice is committed to providing compassionate
care to patients living with advanced, life-threatening illness. Under the
direction of Carolyn Cassin, President and CEO, Continuum Hospice Care provides
comprehensive care for people at the end of life, without regard to race,
creed, disease, sexual orientation, or ethnic background. We actively seek
to serve special populations, such as people living with AIDS or Alzheimer's,
and members of the deaf community. Medicare-certified and licensed to serve
the Bronx, Brooklyn, Manhattan and Queens, we provide care for more than 3,000
patients and their families every year.
Continuum Hospice Care has a full range of services for end-of-life care.
An interdisciplinary team of professionals - physicians, nurses, medical social
workers, therapists, counselors, and volunteers - create an individualized
plan for each patient and family. The staff provides palliative care to ease
the symptoms of chronic pain and serious illness, referrals to needed services
and specialists, and 24-hour on-call physicians and nurses.
Home health aides help patients and caregivers with the tasks of daily living,
and volunteers assist in many ways, from companionship visits to help with
transportation. Grief counseling and bereavement services are available to
anyone in the community who requests our support. When inpatient care is needed,
patients can be admitted to our 16-bed inpatient unit at Beth Israel's Petrie
Division. We have a hydrotherapy room, a family room with kitchenette and
laundry, and a beautiful rooftop garden. Families may stay in the unit to
be near their loved ones.
In addition to the Jacob Perlow Hospice at Beth Israel, Continuum Hospice
Care has expanded its ability to provide inpatient care throughout New York
City, with short-term hospice inpatient care now available at St. Luke's Hospital,
Roosevelt Hospital, Beth Israel King's Highway Division, Long Island College
Hospital, Montefiore Medical Center in the Bronx, and at North General Hospital
in Harlem. We also provide hospice care in many of the city's nursing homes.
In the coming months, we plan to open New York City's first hospice residence
in the Riverdale section of the Bronx. Continuum Hospice Care/Jacob Perlow
